Remote Intensive RN Care Manager (Asthma/COPD/CHF/IBS) need for CT (Work From Home) Exciting work at home opportunity! Established and premier Health Plan for Connecticut's Medicaid eligible residents recognized on both local and national levels is looking for intensive nurse case managers - (Asthma/COPD/CHF/IBS) These positions are field roles 20% and work from home 80% (after training period). The leadership and staff of this Health Plan have a passion to serve Connecticut's population, unmatched by any organization. Intensive Case Manager engages with members in a variety of ways. Care managers communicate with members face-to face and telephonically, conducting assessments, coaching, and mailing educational resources. ICM staff will meet with members out in the community, at the members' homes, shelters, hospitals, skilled nursing facilities, providers' offices, and other community settings Post-COVID. Intensive Care Manager (RN) - (Asthma/COPD/CHF/Irritable Bowel)- Full-time, Monday -Friday, Business Hours We are also recruiting for Intensive RN Care Managers for Behavioral Health and Diabetes/Sickle Cell Disease Several Territories Open in Connecticut - They like the territory within a 30-mile radius near your home. 4 days a week Remote Roles and 1 day a week in the field Caseload of 50 Salary Range: $80k - $85k Seeking professional registered nurse to collaborate with the health plan's clinical team, provider network, State of CT governmental and community-based organizations to make a difference in the lives of some of CT's most vulnerable residents. Achieve professional fulfillment as you support CT Medicaid/HUSKY Health enrollees' achievement of personalized health care goals and navigation of the health care delivery system to get their needs met across the continuum of care. Primary responsibilities include: Acts as a clinical coordinator collaborating with members, providers, facilities and a supportive team of multi-disciplinary colleagues to conduct comprehensive assessments designed to identify the member's medical, behavioral and social determinants of health evaluate member needs Develops a person-centered specific care plan to establish specific goals and overcome barriers identified during the assessment. Develops, monitors and evaluates the effectiveness of the care plan and modifies as necessary. Assists the member by functioning as an advocate by communicating effectively with other members of the healthcare team and by utilizing available resources to promote quality cost effective outcomes and member autonomy. Provides education and coordination of outpatient care to HUSKY Health enrollees identified for Case Management. Identifies and refers quality of care, coordination of benefits and third party liability issues as appropriate. Maintains knowledge of covered benefits for CT Medicaid, HUSKY Health programs. Job Requirements:Qualified Candidates will have the following: Must be a Registered Nurse with at least 4+ years of clinical experience and an active, unrestricted license - 2 years direct clinical patient care experience is required. - Managed care and case management experience preferred. - Recent experience in one of the following areas is desired: Complex adults with co-morbid conditions (Asthma/COPD/CHF/Irritable Bowel). Bilingual preferred, but not required. Requires excellent verbal and written communication skills. - Good time management and organizational skills along with the ability to manage continually changing priorities. - Strong computer and keyboard skills. - Ability to exercise independent and sound judgment in decision making. - CCM or other URAC approved certification preferred or within 4 years of hire. - Must hold a valid and unrestricted motor vehicle operator's license and have reliable transportation. - Salary up to $85,000 (flexible++) + EXCELLENT Benefits!
