Times/Locations Monday - Friday, 8:30AM - 4:30PM; flexibility for
nights and weekends are required. Location: 76 Pliny Street
Hartford, CT 06120 Job Summary: The Clinical Case Manager will work
to support the Mission of My Sisters' Place which is committed to
ending homelessness and empowering women, families, and others in
Greater Hartford to achieve independence and stability in their
communities by providing housing and supportive services. The
Clinical Case Manager holds primary responsibility for working
towards the goal of each client's full and productive integration
with the community. Will work as a member of the Homeless Outreach
Team at CRMHC and be able to provide a full range of intensive
clinical case management services to clients needing assistance to
engage or re-engage with mental health and/or substance abuse
treatment The Case Manager will provide services in a medium sized
nonprofit, multi-cultural, recovery-oriented organization within a
computerized, multi funded (state, local, federal and donor)
environment. This position provides these services for both My
Sisters' Place and Open Hearth. The Clinical Case Manager
interfaces with the Executive Director, Vice President of Programs
& Operations, Assistant Program Supervisors, supervisor, program
staff, volunteers, other organizations, individuals & families and
children of our agency, clients with mental health/substance abuse
issues, CRMHC and Hope Team and Open-Hearth Staff. Duties &
Responsibilities:
- Identify and provide outreach and engagement functions to
shelter and transitional living program residents with mental
health, addiction and co-occurring disorders including
identification of behavioral health concerns, education of
residents in group and individual modalities and assistance in
gaining access to needed are in the behavioral health system.
- Identify clients in need of case management and/or outreach
engagement services and link clients with those services.
- Develop comprehensive assessments (intakes) for clients
eligible for services at CRMHC, present clients for disposition for
CRMHC services.
- Provide clinical consultation and education on behavioral
health issues.
- Provide clinical documentation of client assessments and
contracts as required by CRMHC and MSP and Open Hearth.
- Provides comprehensive planning and active supports for the
care, treatment, and safety of the agency's clients.
- Demonstrates understanding and evaluates clients by
acknowledging individual psychological difficulty, ethnicity, and
history.
- Assists clients with the development of individual plans and
goals and provides supports for achievement.
- Mediates client disagreements and provides crisis
interventions.
- Recognizes varied needs of individuals and plans programs to
accommodate differences and to support a healthy group
dynamic.
- Assists clients with the coordination of health services and,
as necessary, schedules client appointments for and provides
transportation to physicians, clinicians, and other health care
professionals.
- Maintains communication with clients' families and conservators
if appropriate.
- Acts as liaison with other parties and advocates for client
needs.
- Maintains current records, logs and files and completes other
reports and correspondence as necessary.
- Acts as liaison with other agencies, i.e., DSS, SSI, CRMHC, and
other social and vocational agencies to advocate for and assist
clients.
- Assists in maintaining a clean, safe environment including
staff areas and agency vehicles.
- Attends and participates in meetings, conferences, seminars,
and trainings as appropriate.
- Participates with planning activities for clients, program, and
agency.
- Performs other duties as assigned. Supervisory Duties &
Responsibilities:
- This position does not have any supervisory responsibilities.
Education and Experience:
- Master's Degree in Social Work or related field preferred;
plus, three to five years of experience in social work,
rehabilitation therapy, or behavioral science.
- Excellent verbal and written communication skills.
- Clinical background with the ability to work with multicultural
clientele.
- Knowledge of case management principles, practices, and
procedures.
- Considerable knowledge and experience in the dynamics of
individual and group behavior.
- Computer proficiency (i.e., databases, Microsoft Word, Excel),
required.
- Bilingual fluency (English/Spanish) preferred. Certificates and
Licenses
- Valid driver's license, clean driving record and proof of
current automobile insurance, and reliable transportation
required.
- Proof of COVID-19 vaccination required. Salary
- Will commensurate with experience.
