Hospice Liaisons at Landmark are part of a centralized team of
hospice specialists who support community-based clinicians caring
for patients facing serious Illnesses. What makes your career

You will act as a liaison between the Landmark clinical team and
hospice agencies while processing hospice referrals and ensuring a
timely and seamless transition to hospice care. You will provide
education and information about the hospice benefit, as well as
coordinate care for members who elect to receive hospice.
This is a critically important role that creates enormous
satisfaction as you impact the care and quality of life of our
aging population. - All the while, you'll be building meaningful
relationships with patients, families, and providers involved in
the patient's care to maximize value for some of the most
vulnerable individuals in healthcare. Eligible for RRP.
This position is full-time, Monday - Friday. Employees are required
to work our normal business hours of 8:00am - 5:00pm. It may be
necessary, given the business need, to work occasional overtime or
weekends.
*All Telecommuters will be required to adhere to UnitedHealth
Group's Telecommuter Policy.
Primary Responsibilities:
Process all incoming hospice referrals from Landmark providers in
assigned territory
Coordinate accurate and appropriate documentation to facilitate the
hospice admission
Obtain updates on and complete pending hospice referrals within
established timeframe to meet departmental goals
Communicate updates and pertinent information to referring provider
and interdisciplinary team
Prioritize all work, requests, and activities for each referral
Document in real time in the electronic medical record (EMR)
Ubiquity
Hospice monitoring of patients who admit to hospice care for status
updates
Establish and maintain positive working relationships with hospice
providers and Landmark clinicians
Support Landmark providers with hospice transitions and overcoming
barriers to hospice admission including, but not limited to,
hospice eligibility reviews
Complete hospice informational sessions telephonically with
Landmark patients and families. Hospice informational sessions can
include providing education and information about the hospice
benefit, discussing hospice readiness and qualification criteria,
and offering quality hospice providers in the area.
Support Landmark markets as a hospice subject matter expert (SME)
to resolve any hospice or end of life related questions
Assist markets with advance care planning documentation, as
needed

Required Qualifications:
High School Diploma / GED (or higher)
3+ years of hospice experience in a patient facing role focused on
the initial transition to hospice care - must be listed on
resume
Ability to attend 2-3 days in person training the first week, where
the candidate is located, they can flex the training locations.
Ability to work full-time, Monday - Friday. Employees are required
to work our normal business hours of 8:00am - 5:00pm. It may be
necessary, given the business need, to work occasional overtime or
weekends.
Licensed healthcare professional such as Licensed Practical Nurse
(LPN), Social Worker (SW), OR a non-licensed individual with
experience in a high acuity hospice, preferably in a patient facing
role that performed initial hospice conversations, and/or
admissions.
Telecommuting Requirements:
Reside within state of New York, Pennsylvania, Massachusetts,
Connecticut, OR New Jersey
Required to have a dedicated work area established that is
separated from other living areas and provides information
privacy
Ability to keep all company sensitive documents secure (if
applicable)
Must live in a location that can receive a UnitedHealth Group
approved high-speed internet connection or leverage an existing
high-speed internet service
Soft Skills:
Accurate and timely documentation skills.
Excellent written and verbal communication skills.
Detail oriented with good time management skills.
Comfortable working with patients and families at end-of-life
stages.

Connecticut or New York Residents Only: The salary range for
Connecticut / New York residents is $18.80 - $36.78. Pay is based
on several factors including but not limited to education, work
experience, certifications, etc. In addition to your salary,
