Job Description:
A company is looking for an RN Case Manager.
Key Responsibilities
Conduct initial intake calls and ongoing patient outreach to update
care plans and monitor progress
Provide care coordination for a panel of complex patients and
support them during care transitions
Collaborate with a multidisciplinary team to address gaps in care
and ensure patient-centered services
Required Qualifications
Current RN License in good standing in the state of practice
Bachelor's degree in nursing or an associate's in nursing with
another clinical or business bachelor's degree
Minimum of 4 years of clinical experience, with at least 2 years in
case management
Certified Case Manager (CCM) certification or commitment to
complete when eligible
Geriatric care experience is highly desired
