  • As an RN you will case manage patients in the Nursing home setting and respond to symptom management needs of our Hospice patients and coordinate with our team to provide added support to residents and families.
  • Work days are Monday to Friday 8 am to 4:30 pm. NO WEEKENDS!
  • RN must oversee and be collaborative with the nursing home in the care of the hospice patientWhat You Will Need
    • A current license to practice as a Registered Nurse in the State of New York
    • Associates Degree in Nursing, BSN preferred
    • 1 year experience as an RN required. Long term care experience highly preferred.
    • Previous hospice experience preferred but NOT required.
    • Valid Driver's License and Reliable vehicle
    • The RN must be able to communicate effectively, verbally and written

Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
