Job Description Job Title: Senior Care Manager (RN) Location:
Remote - New York, United States Type: Full Time Our Client is
seeking an experienced Registered Nurse to support comprehensive
care management and coordination services. This role focuses on
assessing needs, developing individualized care plans, and
partnering with healthcare providers, members, and caregivers to
promote high-quality, cost-effective outcomes. Key
Responsibilities: Assess, develop, implement, and continuously
evaluate personalized care and service plans Coordinate care
between individuals, caregivers, providers, specialists, and
community resources Monitor progress, changes in condition, and
outcomes; update care plans as needed Identify barriers to care and
recommend interventions for complex or escalated cases Review
referrals, intake assessments, and member data to support quality
improvement initiatives Conduct outreach via phone, digital
platforms, in-home visits, or community settings as appropriate
Collaborate with healthcare providers to facilitate services,
treatments, and care transitions Educate individuals and caregivers
on disease processes, care options, provider instructions, and
healthcare benefits Maintain accurate documentation in compliance
with state, federal, and regulatory standards Partner with
leadership and cross-functional teams to enhance care delivery and
quality outcomes Support onboarding and development of new clinical
team members through coaching and mentoring Qualifications: Degree
from an accredited school of nursing or Bachelor’s degree in
Nursing 4–6 years of relevant clinical or care management
experience Active Registered Nurse (RN) license (state and/or
compact license required)
