Job Description Job Description /n Manages chronically ill
patients through the health care system by assisting with access
issues, developing relationships with service providers, and
tracking interventions and outcomes. /n Job Responsibility /n /n /n
- Provides direct services to patients including the completion of
needs assessments, development of patient focused care plans,
periodic reassessments and overall comprehensive service
coordination. /n - Advocates the requirements of clients within the
agency and with external service providers. /n - Assists providers,
medical assistants, nurses, behavioral health providers, social
workers, etc. by coordinating all aspect of care inclusive of
appointments, referrals, adherence, specialty care, etc. /n - Acts
as the primary channel transmission of information between
providers and patients. /n - Performs related duties as required.

/n /n Job Qualification /n /n /n - Bachelor of Science in Nursing
required. /n - Current License to practice as a Registered
Professional Nurse in New York State required, plus specialized
certifications as needed. /n - 6-8 years of relevant experience and
2-5 years of leadership / management experience, required. /n /n
