SCHEDULED HOURS: 40/hours week; some evenings PROGRAM: Caregiver Support Team SITE LOCATION: Hartford, CT PC#: 1251 ***Competitive salaries and sign on Incentives available*** OBJECTIVE OF POSITION: The Caregiver Support Team Clinician is responsible for all aspects of in-home, community outreach, caregiver support services for DCF referred kinship, foster, pre-adoptive and adoptive families. Other responsibilities include: assessment of service needs of the caregiver and family; implementation of strategies identified in the caregiver support plan for the family; caregiver training; ongoing documentation of assessments and services provided; referral and linkage to additional services or community programs in order to stabilize the living situation and avoid disruption; the establishment of structure in the child's daily routine; and the teaching and role modeling of specialized parenting skills needed to manage challenging behaviors. The Clinician participates as a full member of the team and the program and is responsible for maintaining client records. Screening & Assessment: Conducts brief, evidence-based and developmentally appropriate screening and conducts or arranges for more detailed assessments when indicated. Care Planning & Care Coordination: Creates and/or implements integrated care plans, ensures access to linked services; exchanges information among clients, family members, and providers. Intervention: Provides range of brief, focused prevention, treatment and recovery services for clients. DUTIES & RESPONSIBILITIES: Provides assessment and treatment interventions based on family identified needs. Documents ongoing client progress and outcomes Demonstrates competency in age related or specialty issues and developmental needs for each population served. Provides psycho-education to caregivers about trauma and the impact on developmental stages. Works with clients to develop appropriate discharge plans utilizing natural supports. Maintains collaborative relationships with family members and other service providers Provides crisis assessment and intervention to monitor status of clients in crisis. Responsible for Maintaining Client and Staff Records According to Agency Policy Participates in weekly supervision and meetings to review client and program progress. QUALIFICATIONS: Education: Master's Degree in mental health field Experience: Demonstrated in-depth knowledge and experience as a psychotherapist working with people with mental illness and/or serious emotional disorders. Demonstrated training and experience in all aspects of mental health treatment, assessment, evaluation and provision of individual, group and family therapy. Relevant experience to program model to which individual is assigned. For identified positions, bilingual capacity may be required. Licensure/Certification/Registration: LMSW, LPCA, LMFTA, required. LCSW, LMFT, LPC eligible.
