Clinical Pharmacist (Waterbury)

Job Description:

SUMMARY OF RESPONSIBILITY: Under the general supervision of the Director of Care Management and within established organizational policies and procedures, the Clinical Pharmacist provides comprehensive medication management / optimization services to SOHO-attributed patients by identifying, preventing and resolving medication therapy related problems via communication with providers and patients.   Primary, essential job functions involve ensuring rational drug therapy through interventions that improve health outcomes, reduce cost of care, and increase quality for our patients throughout the continuum of care.

Essential Functions:

  • Provide clinical pharmacy input during the preparation of interdisciplinary care plans for patients
  • Develop and maintain collaborative relationships with physicians, patients, and other healthcare team members within the SOHO network
  • Collaborate with Director of Care Management and Chief Population Health Officer to strategically design and implement a collaborative drug therapy management program within the SOHO network.
  • Provide comprehensive pharmaceutical care for the networks patients, per patient-specific referrals:
    • Review patient records, including allergies, problems, health maintenance, medications and immunization status
    • Elicit and record updates to allergy and medication records
    • Perform patient assessment for medication-related factors
    • Identify any barriers to patient adherence
    • Interpret data related to medication safety and effectiveness
    • Provide information, education, and counseling to patients about medication-related care
    • Document all services in documentation platform and/or patient EHR in a timely manner
    • Participate in multidisciplinary reviews of patients progress
    • Communicate with payers to resolve issues that may impede access to medication therapies
    • Communicate relevant issues to physicians and other healthcare team members
    • Provide individualized health promotion and disease prevention interventions
  • Participate in activities related to improving outcomes for targeted disease management initiatives leading to improved health
  • Provide education and training support to healthcare team in the broad area of pharmacotherapy
  • Effectively communicate with the healthcare team regarding patient care, assessment, and education issues
  • Participate in performance improvement initiatives, population health programs, assigned committees, and other development opportunities established by the organization
  • Collect and maintain data on metrics that will be used to evaluate the impact of pharmacy services on quality measures and patient outcomes
  • Assist in the resolution of complaints, requests and inquiries from network providers and patients
  • Ensure confidentiality of all patient and organizational information in accordance with HIPAA legislation, protected health information (PHI), and organizational policy.


  • Pharmacy degree from a school of pharmacy accredited by the Accreditation Council for Pharmacy Education
  • Pharmacy residency accredited by the American Society of Health-System Pharmacists (community pharmacy, ambulatory care, geriatric pharmacy, or managed care pharmacy focus preferred)
  • Current, unrestricted pharmacist license in CT or MA (dual licensure preferred)
  • Two to five years of recent CMM or MTM experience involving direct patient care
  • Board certified via BPS or NBMTM
  • Valid NPI number
  • Insurability for professional liability
  • BLS certified
  • Proficiency with electronic health records
  • Ability to work independently, exercising sound judgment, discretion, and the initiative to facilitate change
  • Excellent analytical and problem-solving abilities and interpersonal, written, and verbal skills
  • Meets annual continuing education requirements for continued professional license and specialty certification
  • Ability to research and analyze the medical literature including drug information, disease states, and clinical practice guidelines
  • Excellent organizational skills, diplomacy, and professionalism
  • Vision abilities include close vision, color vision, peripheral vision, depth perception and ability to adjust focus
  • Dexterity to utilize a computer keyboard on a regular basis

Physical and Mental Effort:

Physical:  This position requires attendance at meetings at the main office as well as provider office presence throughout a large health care system in a defined geographic area (Hartford, Waterbury, and/or Springfield markets)

Mental:    High-level decision-making and problem-solving skills are required.  This team member will be expected to sort through a variety of viable alternatives to generate the most appropriate solutions for optimal patient care and further the goals of SOHO and as a result may encounter stressful situations.


Environmental and Working Conditions:

Daily work is performed in a combination of work settings, including regional offices, home office, provider offices, and patient homes.  High level of telephone communication, computer use and clinical documentation requirements. Team member may be exposed to sudden fluctuations in temperature, electro-magnetic radiation as in CRTs (VDTs).  Team member will be required to travel to different sites; as such, the position requires use of a personal vehicle for frequent movement within a defined geographic region. There is a potential for exposure to infectious patients, materials and hazardous chemicals (e.g. cleaning supplies) in provider offices.
