Bilingual Case Manager, JRB

Job DescriptionRESPONSIBILITIES: Provide initial screening and triage.Conduct home visits to youth and family.Provide comprehensive, intensive case management services to JRB youth and family.Obtain informed consent of parent/youth to participate in the JRB.Obtain consent of victims in delinquency matters.Administer identified assessment inventory.Collect family, school and other relevant history.Develop packets summarizing assessment results and fact finding for each youth.Develop written individualized plan for each student with specification steps, time frames and responsibilities.Present cases to Juvenile Review Boards for arrest cases.Serve as each family's designated advocate within the school system, the court system, and wherever else needed.Arrange for any community resources linking youth and family to recommended programs/services according to youth's Diversion Contract or service needs.Monitor and document youth's progress.Create and monitor an aftercare plan, if needed, for a student upon student's completion of the program.Notify youth and parent/guardian of discharge from JRB process.Experience and SkillsQUALIFICATIONS:Bachelor's degree in Human Services or related field required.1 year experience working with at-risk youth or truant youth.Must have current valid state issued Driver's License.Must have own vehicle and be willing to drive regularly to home visits and other appointments within the city of Hartford.Proficient with word processing and spreadsheet applications. Possess solid general computer skills, including ability to work in a Windows XP environment. Familiarity with specific applications used by the Village a plus.Knowledge of the juvenile justice system and school system a plus.Must be a team player.Must be culturally competent to serve diverse client population; bi-lingual (English/Spanish) preferred.Physically able to perform the essential functions of the position, with or without reasonable accommodations.
