SUMMARY The Fetal Care Center at Connecticut Children's understands the emotions parents experience when they learn their infant has a complex medical condition. The Fetal Care Nurse Coordinator, also referred to as a nurse navigator, serve as the initial point of contact for all appointments from the first visit, throughout the pregnancy, and after delivery. The Fetal Care Nurse Coordinator serves as a single point of contact for expecting mothers navigating their specialty appointments. They also provide comprehensive patient education, support and help to ensure continuity of care. --- On the day of the patient's appointment, the Fetal Care Nurse Coordinator will accompany the parents throughout the day and answer any questions he or she may have. These appointments and diagnostics tests may include the following, depending on the infant's diagnosis: * Fetal MRI * Fetal ultrasound * Fetal echocardiogram * Appointments with pediatric subspecialists The Fetal Care Nurse Coordinator will also schedule follow-up appointments with subspecialists and answer any questions throughout the pregnancy and after delivery. Our Fetal Care Center Nurse Coordinator's role is to be available to coordinate appointments and consultations within the center and the hospital's pediatric specialties. The goal is to help the parents feel fully prepared for their baby's medical care after birth. Under indirect supervision, coordinates and provides comprehensive patient care and associated services within a defined specialty practice. Assesses patient/family needs; coordinates care and associated procedures or services for complex patients; provides education to patient/family; provides telephone advice to patient/family, based upon protocol; functions as an advocate and resource for patient/family & health care team; and facilitates communication within health care team. Mentors staff & delegates tasks to staff within the practice, collaborates with other health care providers, schools and/or community services to meet patient/family needs. ROLE RESPONSIBILITIES Prioritizes, plans, provides direct family-centered care to patients within area of service throughout the continuum of care utilizing time and resources effectively and efficiently. Facilitates the flow of patients within the clinical practice.Partners with child and family in evaluating outcomes. Monitors, trends and evaluates child/family responses to treatment throughout scope of service.Provides appropriate education to patient/family regarding all aspects of care, including telephone advice, based upon established protocols. Serves as advocate for parents and families.Clearly documents all pertinent aspects of care, including: a plan of care; a regular summary of patient status and family adjustment; unexpected events of changes in patient condition; medications; and monitoring of vital signs, etc. Complies with CCMC policies regarding appropriate documentation.Adheres to current HIPAA Guideline to insure patient and family confidentiality is maintained.Delegates aspects of care to peers and ancillary personnel to meet needs. Serves as a resource to staff within area of expertise. Networks with healthcare team by documenting and communicating comprehensive data including complex child/family needs to provide continuity.Demonstrates knowledge of the age-related differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice. Demonstrates cultural sensitivity in all interactions with patients/families. Values cultural diversity and other individual differences in the workforce. Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent with the CT Children's standards.Performs other related responsibilities as assigned. Travel to satellite locations o support departmental demands as needed. Graduate of an accredited Registered Nurse Program.
