Working closely with the Risk Adjustment and Quality Strategy and Operations teams, the Practice Performance Improvement Advisor will integrate Evolent programs and resources within local practice workflows with the intent to optimize improvement opportunities, positively impact physician engagement and the quality of care provided. The Practice Performance Improvement Advisor will also assist with regional operational duties as needed, with direction from the Strategy team.What Youll Be Doing:The Practice Performance Improvement Advisor will travel throughout their designated territory to build relationships with physician practices to support physician engagement, clinical practice transformation and performance improvement by conducting the key functions below:Guiding actionable initiatives from risk adjustment, quality, and clinical data; measuring progress and outcomes and helping implement interventions where applicableIntegrating care management, utilization management, risk adjustment, and quality initiatives within the practice workflows and ensuring goals are met by assigned practicesLeading the transition of innovative ideas from pilot to implementation and sharing best practices across all practices and physician governance structuresIn collaboration with cross-functional teams, helping develop content for physician/practice manager forums/meetings and presenting a compelling performance strategyBuilding strong relationships by working with go-to physician leaders and leveraging practice relationships to engage, implement, and sustain Partner prioritiesServing as primary liaison to practice managers and staff for assigned practicesIn collaboration with the Strategy Lead, developing engagement strategy for practice managers and clinical liaisons in assigned territoryProviding training and education to practices as neededServing as a local Evolent liaison for the Evolent Market Operations team as needed, assisting with local communications efforts, meeting, and event coordination, etc.Building relationships with and serve as liaison to community resources and/or ancillary providers, skilled nursing facilities, etc. as neededThe Experience Youll Need (Required):Bachelors Degree or equivalent work experience5+ years of leadership experience in practice management, provider relations and project management2+ years of data analysis and interpretation of reports for risk adjustment or quality performanceDemonstrated experience working with physicians and provider practices to drive improvement in risk adjustment and quality performanceMust have a valid Driver's License and ability to travel locally/regionally up to 75% of the work weekDemonstrated skilled proficiency in Microsoft Office products (Excel and PowerPoint)Excellent written and oral presentation skills, with the ability to engage, inspire, build credibility and trustHighly organized and self-motivated individual with ability to adapt to various workspaces and work autonomouslyCollaborative working style with the ability to work across different teams, areas of expertise, and adapt to ambiguous environments and clienteleHypothesis driven to identify trends, predict issues, highlight critical areas, and develop corrective action plansFinishing Touches (Preferred):3+ years experience in practice re-design work including Patient Centered Medical Home, Practice Transformation, Quality Improvement, Risk Adjustment, Ambulatory Care, Quality and Efficiency metrics, and EMRHealth plan or managed care experience with a provider perspective and strong orientation to value-based care principles and the health policy landscapeWorking knowledge of clinical quality metrics such as Risk Adjustment, HEDIS, NCQA or National Quality Forum type metricsTechnical Requirements:Currently, Evolent employees work remotely temporarily due to COVID-19. As such, we require that all employees have the following technical capability at their home: High speed internet over 10 Mbps and, specifically for all call center employees, the ability to plug in directly to the home internet router. These at-home technical requirements are subject to change with any scheduled re-opening of our office locations.Evolent Health is committed to the safety and wellbeing of all its employees, partners and patients and complies with all applicable local, state, and federal law regarding COVID health and vaccination requirements. Salaries are determined by the skill set required for the position and commensurate with experience and may vary above and below the stated amounts.
