Case Manager VEVRAA Federal Contractor: Priority Referrals of
Protected Veterans Requested Columbus House opened its doors in
1982 to provide services for men and women at least 18 years of
age. Our goals quickly broadened from the mere provision of food
and shelter and simple survival, to understanding and working
toward overcoming the problems which cause people to become
homeless. Columbus House, along with its core of loyal supporters,
friends and volunteers, has remained committed to fulfilling its
mission: "To serve people experiencing homelessness or at imminent
risk by providing life-saving outreach, shelter and housing and by
fostering their personal growth and independence. We advocate for
and create affordable housing to end homelessness in our
communities." Purpose Under the direction of the Medical
Respite/Recovery House Manager, the Medical Respite Case Manager
provides supervision, safety, organization, and case management
support to twelve clients at the Medical Respite unit. Services are
of limited duration; 30 to 60 days. The CM networks with the
greater New Haven shelters, sober houses and social service system
and collaborates with YNHH, clinical social workers, unit nurses
and other Columbus House residential staff to provide an
environment that reflects the core values of Trauma Informed
Service: Safety, Trustworthiness, Choice, Collaboration, and
empowerment. Duties and Responsibilities • Excellent organizational
and time management skills with a deadline-oriented mindset •
proficient is Microsoft Office, Word, HMIS database. •
Knowledgeable about health and safety, substance use, harm
reduction, crisis intervention, dual diagnosis. • Great
interpersonal skills • Ability to collaborate with other team
members, and community agencies and Partners. • Ability to develop
partnership with clients. Employees should be able to create a safe
and welcoming place for the clients that we serve. Qualifications,
Knowledge, Skills and Abilities • Associate degree or bachelor’s
degree, preferably. • Demonstrated the ability to manage, motivate
and direct individuals living with acute medical conditions,
substance abuse and mental health. • Complete knowledge base of all
aspects of the delivery of residential services for the homeless
population. • Excellent organizational and time management skills
with a deadline-oriented mindset • Proficient is Microsoft Office
and other common desktop applications. • Able to effectively use
the local network of substance abuse, mental health, and social
services providers. • Familiar with mental health, substance abuse
and dual diagnosis issues and group dynamics • Knowledgeable of
local housing and vocational/entitlement programs • Culturally
competent; must possess the capacity to work within a team and
foster teamwork. • General understanding of crisis intervention
skills, confidentiality, boundaries, universal precautions • Able
to meet all documentation requirements. • Able to foster a Harm
Reduction philosophy within the work environment. • Strong empathic
skills coupled with a high degree of reliability and punctuality. •
Valid drivers’ license with a safe driving history Supervision:
Medical Respite Recovery House Manager Salary Range: $25.50 to
$27.50 per hour plus excellent benefits Schedule: Monday – Thursday
8:00 a.m. to 4:00 p.m. Benefits • Medical, dental, and vision
