Job Description Job Description Hospice Social Worker (MSW) Acclaim Home Health is seeking a compassionate and dedicated Hospice Social Worker (MSW) to join our interdisciplinary hospice care team in Worcester, MA. The Hospice Social Worker plays a vital role in supporting patients and families through end-of-life care by providing emotional support, counseling, care coordination, and access to community resources. Responsibilities Provide psychosocial assessments, counseling, and emotional support to hospice patients and their families Assist patients and caregivers with coping strategies related to terminal illness, grief, and end-of-life planning Coordinate with the interdisciplinary hospice team (nursing, chaplaincy, aides, physicians) to develop and implement patient-centered care plans Connect patients and families with appropriate community resources, financial assistance, and support services Participate in care conferences, team meetings, and discharge planning as needed Provide crisis intervention and bereavement support Maintain accurate and timely documentation in accordance with hospice regulations and company policies Ensure compliance with state, federal, and hospice regulatory requirements Qualifications Master’s Degree in Social Work (MSW) – REQUIRED Current Massachusetts Social Worker license (or eligibility to obtain) Hospice or healthcare social work experience preferred Strong communication, counseling, and organizational skills Ability to work independently and as part of a collaborative care team Compassionate, patient-focused approach to care Reliable transportation and ability to travel locally as needed Why Join Acclaim Home Health?
