Street Outreach Case Manager, your mission is to help individuals
facing mental health or substance use challenges secure stable
housing and rebuild their lives. If you’re passionate about making

you’ll be out in the community—on the streets, in shelters,
libraries, emergency rooms, and other public spaces—meeting people
where they are. You’ll work closely with service providers to break
down barriers, open doors to vital resources, and help individuals
take the next step toward stability and independence. Location :
Hartford, CT Schedule: 40 hours per week, flexible days/hours – but
expect at least three evening/weekend shifts for street outreach.
Hours : Monday-Friday 7am-3pm Temporarily, hours are subject to
change seasonally. Schedule is subject to change to accommodate

combating homelessness and providing support to individuals and
families in need. We provide safe housing, life-changing services
that include medical care, management of financial affairs, short-
and long-term housing and all the necessary tools to reclaim their
independence. At CHA, you will be part of a team that truly makes a
difference in the lives of those who need it most. What You’ll Be
Doing · Build trust and connect individuals with services. ·
Provide hands-on support to connect people with safe shelter and
long-term housing solutions. · Spearhead projects and initiatives
with Greater Hartford Coordinates Access Network (CAN) and other
community partners to bridge gaps in services. · Help clients
access mental health, substance use treatment, and healthcare
services. · Keep accurate records (data entry in CT HMIS) to track
progress and ensure accountability. · Be a team player,
collaborating with our Homeless Services Navigator to support
individuals from homelessness to housing stability. · Conduct
street outreach in all weather conditions—because the need doesn’t
stop when it rains! In this role you are actively involved in
changing someone’s life for the better. You will be an advocate, a
beacon of hope, working with service providers to break down
barriers and open doors for many. What We’re Looking For: ·
Flexibility – No two days are the same, and you’re ready for that!
· A self-starter mindset – You thrive in the field and can make
quick decisions independently. · Strong interpersonal skills –
You’re great at building trust and setting healthy boundaries. ·
Comfort with technology – You’ll need to document interactions and
track client progress. · Firsthand understanding of homelessness
and/or behavioral health (lived experience is a plus!). · Physical
ability to carry supplies and assist with client needs (lifting up
to 20 lbs). Requirements: · Bachelor’s degree (B.A.) from an
accredited college or university, preferably in Social Work or a
related Human Services field (preferred). · Familiarity with
homeless outreach and the Housing First model. · Experience in
conflict resolution and mediation. · At least two (2) years of
relevant work experience with a proven ability to effectively
support clients. · Strong cultural competency to work with diverse
populations. · Bilingual candidates are a plus! Licenses and
Certifications · Must have a valid Connecticut driver’s license and
clean driving record with proof of automobile insurance. What You
